The Home Health Team
The Southern California Home Health teams works closely with patients, families and physicians to assist patients on the road to recovery. We have registered nurses (RNs), licensed vocational nurses (LVNs), physical therapists, occupational therapists, speech therapists and certified nursing assistants (CNAs)/home health aides. Our nurses assist with numerous treatments, such as wound care, wound vac, catheter maintenance, ostomy care, lab work and IV Infusion therapy.

Services We Provide
Palliative Care
This is a special health service that focuses on improving quality of life for patients and families coping with serious illness.
Physical Therapy
Our physical therapists design and oversee rehabilitation programs aimed at restoring skill for functional mobility. We can help patients overcome disabilities caused by cardiac arrest, neurological and orthopedic conditions and traumatic injury. We can assist with:
- Bed mobility
- Transferring
- Balance and ambulation problems
- Generalized weakness or fatigue
- Pain management
- Utilization of adaptive device and safety education
Occupational Therapy
Occupational therapists at Southern California Home Health work with patients to help improve their ability to perform their activities of daily living (ADLs). Exercises and activities are designed to aid the patients with tasks such as eating, grooming, and dressing for upper and lower extremities. Our CNAs and home health aides work closely with RNs, LVNs and all the disciplines of therapy services. They help plan, implement and supervise a therapeutic program to improve the patient's level of independence in activities of daily living lost due to illness or injury.
Skilled Nursing
Our skilled nursing services include wound care, IV therapy, medication reconciliation, pain management and education for postoperative health issues.
Medical Social Services (MSW)
We provide medical social services including counseling services for patients and family caregivers, assessments of pertinent psychosocial and economic factors, and identifying and obtaining community resources.
Other services provided by Southern California Home Health include dietary counseling, patient and family education, and interdisciplinary case management.
