Medical Record Requests
The Health Information Management department maintains records of the care patients receive while they are at Corona Regional Medical Center. As required by state and federal law, these records are kept in strict confidence and are only released with proper authorization. We allow multiple ways for patients to obtain medical records and radiology studies.

Fees
Processing and copy fees may be applicable to your request. Costs will vary depending on the type of request. There are no fees associated with the patient portal, Health Records Online.

How may I request medical records?
Accessing your Patient Portal is the fastest way to obtain your health information if you are an adult. If the patient is a minor, the fastest way to obtain the health information is by submitting an Online Request above.

What are your hours of operation?
Hours are 8:30 am to 4 pm Pacific Standard Time, Monday through Friday. The office is closed on weekends and national holidays.
For Corona and Palmdale: Hours are 8 am to 4 pm Pacific Standard Time, Monday through Friday. The office is closed on weekends and national holidays.
Who can request medical records?
Only the patient, duly authorized representative or other healthcare provider currently treating the patient may request medical records. Another person or entity may only receive copies of a medical record with a proper consent form authorizing the release of information.
May my spouse request and receive copies of my medical records?
No. Another person may not receive copies of your medical record without you consenting to authorize us to release the information. If a duly authorized representative is requesting copies of medical records, then a legal document must accompany the request.
How long does it take to get records?
California law allows us up to 15 business days, however, we aim to process requests as quickly as possible.
How can I receive my medical records?
Immediate access through the Patient Portal. Requesting through medical records online will be delivered via your preference, i.e., encrypted email, mail.
How do I request a birth certificate?
You may request a copy from Riverside County, Vital Statistics at 951-358-5068, or from Los Angeles County at 213-288-7812.
May I request records over the phone?
Due to patient confidentiality and information security issues we do not accept telephone requests. We need a written and signed authorization to release protected health information.
